Reverse-transcription polymerase chain reaction (RT-PCR) of tients of the former group remained PCR during the followup and relapsed at a median time of 5 months (range, 2 to the PML/RARa fusion gene may predict relapse in acute promyelocytic leukemia (APL) patients in hematologic complete 9) from ABMT and 9 months (range, 4 to 14) from second CR. Of the eight PCR patients, all remained PCR during remission (CR). We have prospectively studied by RT-PCR 15 PML/RARa APL patients undergoing autologous bone the follow-up controls. One patient relapsed at 10 months from ABMT, one died of a secondary (PML/RARa) leukemia, marrow transplantation (ABMT) in second CR. The median time of first CR duration was 12 months (range, 6 to 40). All and six are in hematologic and molecular remission at a median time of 28 months (range, 15 to 60) after ABMT and patients were reinduced with all-trans retinoic acid (ATRA), followed in 12 of 15 cases by mitoxantrone and Ara-C as 32 months (range, 17 to 62) from second CR. Our results indicate that, in APL patients in second CR, ABMT with PML/ consolidation. Fourteen patients received the BAVC (BCNU, Ara-C, m-AMSA, and VP-16) schedule as conditioning regiRARa marrow cells is likely to result in prolonged clinical and molecular remissions. Conversely, patients who test men. Unpurged marrows were collected immediately before conditioning treatment, analyzed by RT-PCR, and reinfused PCR after reinduction necessitate the use of alternative aggressive approaches, including unrelated allogeneic transat median of 2 months (range, 2 to 7) from the achievement of second CR. Absence of reverse transcription-polymerase chain reaction detectable residual disease in patients with acute promyelocytic leukemia in long-term remission.

Hybrid fusion genes are specific tumor markers of several leukemic subtypes. The use of reverse transcription-polymerase chain reaction (RT-PCR) to amplify chimeric cDNAs allows sensitive detection of the neoplastic clone for diagnostic and monitoring studies in these leukemias. Nonetheless, the clinical relevance of minimal residual disease (MRD) evaluation by PCR remains controversial. In thi...

Cytogenetic and FMS-Like Tyrosine Kinase 3 Mutation Analyses in Acute Promyelocytic Leukemia Patients

Background: The secondary genetic changes other than the promyelocytic leukemia-retinoic acid receptor (PML-RARA) fusion gene may contribute to the acute promyelocytic leukemogenesis. Chromosomal alterations and mutation of FLT3 (FMS-like tyrosine kinase 3) tyrosine kinase receptor are the frequent genetic alterations in acute myeloid leukemia. However, the prognostic significance of FLT3 mutat...

The PRKAR1A gene is fused to RARA in a new variant acute promyelocytic leukemia.

We report the molecular and cytogenetic characterization of a novel variant of acute promyelocytic leukemia (APL). The bone marrow showed 88% hypergranular promyelocytes, and the karyotype was 47,XY,+22 [5]/46,XY[30]. Fluorescence in situ hybridization (FISH) indicated disruption and deletion of the 5'-end of the RARA gene. Treatment with all-trans retinoic acid, idarubicin, and arsenic trioxid...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Therapy of Molecular Relapse in Acute Promyelocytic Leukemia

Fourteen patients with PML/RARa-positive acute promyelocytic leukemia (APL) were given salvage therapy at the time of first molecular relapse. All patients had achieved first molecular remission after the AIDA protocol (all-trans retinoic acid [ATRA] 1 idarubicin) and were being prospectively monitored by reverse transcriptase-polymerase chain reaction (RT-PCR). Molecular relapse was defined as...

Acute Promyelocytic Leukemia: Clinical Relevance of Two Major PML-RARa Isoforms and Detection of Minimal Residual Disease by Retrotranscriptase/ Polymerase Chain Reaction to Predict Relapse

Recent data have shown that the PML-RARa fusion gene resulting from translocation t(l5;17) isa highlyreliable molecular marker of acute promyelocytic leukemia (APL). In this study performed on 97 Chinese patients with APL, the retrotranscriptase/polymerase chain reaction (RT/PCR) was used to evaluate the clinical relevance of the long (L) or short (S) PML-RARa fusion mRNA isoforms and to study ...
